I was doing the Irina quest where you go talk to her father and kill the boss for the sword he needs to protect. You are supposed to talk to the father first and give him the letter, which he then tells you he has to protect the sword of the castle which the boss at the end of the castle has. I, instead of talking to him first killed the boss and THAN talked to him.

So, I completed Castle Morne, defeated the boss, delivered the letter to Irina's father, went back to Irina, found her dead along with her father mourning.
