For some reason, I got through all 4 pages but when I tried to digital sign, it was defaulting to dates from earlier this week. It wouldn't allow me to check my travel date as Thursday, 19 November 2020.Easiest and quickest is to get either a rapid antigen test or rapid diagnostic test at either Walgreens or CVS. Both are acceptable and quicker than a PCR test (and only an easy nasal swab, not a brain probe LOL).
